The Case Manager will aid with managing the day to day Service Coordination and Case Management operations for Serenity Care Resources LLC. Must be proficient in supporting all administrative functions of Case Management, the Maximus Process and retention for all clients. The role requires interacting appropriately with elderly and disabled clients who have receive services SCR. The Case Manager must be an outgoing person with a caring, compassionate heart who enjoys talking with others and making a difference in people's lives. The Case Manager must focus on the growth and retention of prospective clients for the agency. The Case Manager may also assist with tabling events, marketing, social media campaigns and other aspects of business promotion in addition to their case management duties.

Reports To: Administrator

Essential Functions:

  • Driving to and from Client Assessments
  • Acute understanding of the Maximus enrollment process
  • Retention, on-boarding and Case Management for all prospective and internal clients
  • Provide comprehensive services available to support people who are living with disabilities, aging, or who are recovering from an injury.
  • A Case Manager with Serenity Care Resources will effectively manage a case load of Participants and insure Participant services in accordance with the assigned waiver program requirements.
  • Identifies and monitors the services provided to Participants through the Office of Long-Term Living (OLTL) to determine quality of services provided.
  • Must be able to verify if: Type, amount, scope, duration and/or frequency of services need to be adjusted.
  • In partnership with the Participant, the Case Manager will support goals to live in the most integrated community setting as independently as possible and to establish natural community connections.
  • The Case Manager prepares all necessary forms that allow the Participant to continue to receive services in the time frame established by program guidelines and standards.
  • The Case Manager provides necessary training and support to Participants and Caregivers in areas of client care and safety.
  • Completion of Daily Activity Report and Allocations to assure appropriate program billing/allocation is met.
  • Building relationships with external vendors such as hospitals, Doctors offices, service coordinators and social workers to generate leads to Serenity Care Resources
  • Work with billing Coordinator to ensure that all clients maintain active Medicaid and MCO coverages
  • Follow Up On Leads In The System
  • Other Duties as assigned

Required:
  • Valid Driver's License

Preferred Qualifications:
  • A 4-year college degree is preferred for this position.
  • At least 2 years experience in case management and/or office coordination/management
