Job Description

JOB PURPOSE:

Care Ring an independent non-profit healthcare agency serving the Charlotte community since 1955 is seeking a Nurse Practitioner (NP) for Care Coordination Services. The primary role of the NP is to review medical charts of pregnant and interconceptional clients to assess health and reproductive needs. In-addition, the NP will link clients to community resources to address social determinates of health and assist with attaining targeted goals through education and support. Working collaboratively with the Medical Director or care team as appropriate, the NP provides preventive and well women care in the Low-Cost clinic to address non-pregnancy health and medical related concerns. The NP provides medical services within the scope of licensing, training, and credentialing.

RESPONSIBILITIES / ACCOUNTABILITIES:

Basic Job Functions:

  • Sees clients of all ages during clinic hours set by agency.
  • Practices medicine within the scope of licensure, training, and credentialing.
  • Practices medicine in accordance with current standards of care.
  • Practices medicine in accordance with the quality criteria and procedures adopted by the clinic.
  • Communicates with other providers/staff regarding patient care as needed.
  • Demonstrates the ability to assess, plan, implement and evaluate individual client care appropriate to the age of clients served by demonstrating knowledge of the principles of growth and development over the life span.

Administrative:

  • Communicates appropriately with the client regardless of their age and education level.
  • Works independently while retaining willingness to seek MD assistance as necessary.
  • Reviews labs in a timely fashion.
  • Refers patients to specialized healthcare resources for consultation/treatment as appropriate.
  • Meets/speaks with collaborating physician in accordance with NC guidelines.
  • Maintains patients' medical records in Epic according to established standards and documents in a systematic, accurate and succinct format. Completes office progress notes within 48 business hours.
  • Participates in the development/review of clinic policies and supports the COO in her/his duties.
  • Maintains an environment of safety for patients, self and others.
  • Interact with clients, staff, and partners in a respectful manner at all times with discretion, respecting client confidentiality.
  • Maintains licensure and credentialing, completing annual CME requirements.
  • Performs other duties as required or assigned

ESSENTIAL JOB FUNCTIONS:

  • Comfort and familiarity with working with low-income/disparate populations and communities
  • Accurate medication prescriptions
  • Documentation in Epic
  • Ability to exercise independent judgment and reason objectively.
  • Ability to communicate effectively in written and oral expression
  • Ability to document accurately, concisely and in a timely manner.
  • Ability to handle a variety of duties

POSITION REQUIREMENTS:

  • Excellent written and verbal communication skills
  • One year (1) of case management experience in community, educational or medical environment.
  • Excellent clinical assessment skills
  • Ability to work some evenings and Saturdays
  • Completes annual HIPPA and BBP modules in accordance with guidelines
  • Maintains BLS certification
  • Receives annual TB screening and flu vaccine

MINIMUM REQUIREMENTS:

Education:

  • Accredited school of licensure for Nurse Practitioner or Physician's Assistant
  • Unrestricted license to practice in NC

Certifications:

  • Current DEA Certification

Knowledge and Skills:

  • Excellent clinical assessment skills.
  • Excellent written and verbal communication skills.

PREFERENCES:

  • Bilingual English/Spanish preferred
  • Clinic experience with low-income clients preferred
  • Experience with Epic in the ambulatory setting preferred
