POSITION SUMMARY

The Case Manager II recruits, screens for eligibility, conducts intake services, and assists the Community Services Director in quality monitoring activities for low-income individuals and families in Burke, Caldwell, and Rutherford counties for the purpose of educating those families about the availability of resources to meet their needs. The Case Manager II assists individuals utilizing a variety of case management tools for Information and Referral, Self-Sufficiency, Circles, CARES, Head Start, Weatherization and the Senior Services programs.

               

ESSENTIAL FUNCTIONS

Level of Performance:


  • The employee is expected to demonstrate exceptional interpersonal and intuitive skills to help adult customers to mobilize internal and external resources to improve their economic status.
  • Interview clients to determine need and eligibility for services. Provide orientation on the program mission.  Collect individual assessments of the situation: employment, education, family status; housing, transportation, nutrition, etc.
  • Assist adult clients to prepare an individual and family social skill development plan, setting goals, and establishing outcomes as measurements of progress toward objectives.  Provides counseling, makes referrals, etc. to facilitate appropriate decision making.  Maintain records and document case notes of participant progress, workshop participation, and other services provided.  Counsels participants and refers to other agencies as needed for counseling and services.
  •  Make appropriate referral for services based on the result of the assessment and client needs; complete and/or assist with completion of the application for services for identified services.
  • Advocate and maintain contact with individuals and family throughout program participation via all types of communication including in-person meetings at the client’s home, office, and alternate locations.  Provide advice and assistance in achieving goals.  Follow up on all active and inactive cases in the state reporting system (AR4CA)


Follow-up on the progress of the case management service plan to determine the quantity of service provided to the client and the status of the client’s case.  Maintains adequate and up-to-date records on activities performed and clients served, pending applications, etc. Obtains supplemental documents as needed.

  •  Identify resources to be supplied by BRCA and those from other agencies.  Track expenses, services, progress toward stated objectives, etc., and enter into data base.
  • Prepare monthly outcome reports and evaluations.  Submit reports to Community Economic Services Program Manager.  Prepare related reports as directed by supervisor.
  • Generate collaborative relationships among service providers, identifying unmet needs and acting as an advocate of low wealth individuals and families in the local community. Actively serve on community boards, councils and committees that provide services to agency clients.
  • Assist in researching for resources and funding to supplement CSBG funding and services.  Participate in preparation of annual refunding application.
  •  Facilitates the use of research data to improve existing programs and utilization or resources.                 
  • Perform related duties as assigned by supervisor.  Perform all duties in accordance with agency policies and procedures, and state or local regulations and in compliance with best practice standards.

Education

  • Bachelor's Degree (four-year college or university) in Community Service, Social Work, Human Services or related field.


Experience

  • Two to four years professional experience working with economically disadvantaged individuals and families; or equivalent combination of education and experience, preferred or equivalent work experience.

 

 Skills & Abilites

  • To perform this job successfully, an individual should have a working knowledge of Internet and MS Office.  Must be able to learn program software reporting systems within 90 days of employment to input and monitor data and prepare required documents.
  • Bilingual ( English and Spanish) preferred
