The Director of Care Coordination is accountable for the leadership and oversight of Care Coordination Programs and implementation of new projects related to care coordination, community health programming, and health equity. This position leads a team of managers, nurses, social workers, and community health workers to support residents across the South Side of Chicago. The Director will serve as a liaison among key internal and external stakeholders, and ensure effective implementation, ongoing monitoring, and evaluation of programs across the collaborative.
Responsibilities:
- Provide leadership for care coordination staff and care coordination program administration.
- Provide support and direction to managers to ensure appropriate scheduling, resource allocation and evaluation of work products/activities.
- Ensure care coordination staff are properly trained, integrated within their care teams, and productive in their outcomes and quality of work.
- Ensure staff have adequate training and skills to engage patients and community members.
- Lead program recruitment efforts through developing community partnerships, referral pathways, marketing, and community outreach.
- Develop partnerships and maintain ongoing communication with healthcare providers and social service providers to remove barriers in accessing health care and addressing social needs.
- Lead the development of new workflows for projects and programs related to Care Coordination, with a focus on social services, population health, and the social determinants of health.
- Support and collaborate with data management and IT/IS teams to ensure effective and efficient processes to collect program performance metrics.
- Report program activity and progress regularly to Executive Team, project stakeholders, and partner organizations.
- Develop a process to review program policies and procedures and modify periodically as necessary, as well as quality assurance practices.
- Other duties as assigned within the scope of position expectations.
- Local travel to healthcare organizations – safety net hospitals, health systems and Federally Qualified Health Centers — on the South Side of Chicago required.

Education and Qualifications:
- Master’s Degree in Social Work, Nursing, Public Health, or related field preferred.
- Minimum seven (7) years of experience in community health, care coordination/case management of patients of various risk groups required.
- Minimum three (4) years of managerial experience in a healthcare setting required.
- Strong leadership and mentoring skills.
- Two (2) years of experience working in an Accountable Care Organizations (ACO), Clinically Integrated Networks (CIN), Managed Care Organization (MCO) or provider organization that was compensated under advanced payment model (APM) contracts preferred.
- Demonstrated success with the application of formal performance improvement methodologies (e.g. six sigma, lean, etc.) preferred.
- Program design and development experience.
- Strong ability to organize and prioritize projects and program objectives.
- Ability to work collaboratively within and outside of the organization, and to cultivate partnerships.
- Strong program management and leadership experience.
- Experience managing grants.
- Excellent written and verbal communication skills.
- Proficient in Microsoft Office applications.
