Position Overview:

A dedicated and compassionate Community Outreach Case Manager is an important part of our team. The ideal Case manager will have a strong background in social work or related fields, with a passion for helping individuals overcome challenges and achieve their full potential. The Community Outreach Case Manager will be responsible for providing support, resources, and advocacy to individuals and families facing various socio-economic, health, and environmental challenges.


Job Summary

The Case Manager provides case management services in the community to patients experiencing serious mental illness and/or those who are considered high risk due to their acuity level. The Case Manager assists in planning, connecting, advocacy, coordination, and monitoring; to assist patients in gaining access to health services, financial assistance, housing, employment, education, social services and natural supports. They assess needs, develop and monitor individualized treatment plans and document services provided in an accurate and timely manner. The Case Manager embraces the recovery philosophy and promotes a safe recovery environment for clients. The Case Manager will be responsible for implementing advanced techniques and interventions to serve the high-risk individuals in their associated program.


Responsibilities:

  • Conduct outreach activities to identify individuals and families in need of support within the community.
  • Assess the needs and strengths of clients through comprehensive intake interviews and assessments.
  • Develop individualized care plans in collaboration with clients, setting goals and identifying appropriate interventions.
  • Provide ongoing case management support to clients, including referrals to internal and external resources such as healthcare services, housing assistance, employment opportunities, and educational programs.
  • Advocate on behalf of clients to ensure access to necessary services and resources, and address barriers to care.
  • Monitor client progress and adjust treatment care plans as needed.
  • Collaborate with community organizations and government agencies to coordinate services and maximize resources for clients.
  • Maintain accurate and up-to-date client records and documentation in compliance with organizational and regulatory requirements.
  • Participate in team meetings, trainings, and professional development activities as required.
  • Contribute to program evaluation and quality improvement initiatives to enhance service delivery and outcomes.

Skills/Requirements:

  • High school diploma or GED required. Associate degree (or higher), in a field related to Behavioral Health is preferred but not required of this position.
  • 1 year of experience in a behavioral health position providing outreach and engagement activities.
  • Or equivalent combination of education and experience.
  • 1 or more consecutive years in recovery from Alcohol or Substances and/or GMH is preferred.
  • Or equivalent combination of education and experience.
  • Current AZ Driver's License valid and in good standing
  • Clear 39-month Motor Vehicle Record (MVR)
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and Sanctuary Recovery Centers Policies and Procedures.
