Your Mission:

As the Community Health Navigator Manager (CHNM) reporting to the Associate Director of Human Services, you will lead our community health navigation team, oversee program operations, and identify opportunities to excel in community health navigation. Your role is instrumental in achieving our mission and vision, focusing on inclusivity, empowerment, collaboration, transformation, and resiliency.

Your Impact:

As a Community Health Navigator Manager you will:

Leadership:
  • Plan, organize, and lead the community health navigation program to ensure high-quality service delivery.
  • Collaborate with the Adult Services Department leadership team to establish and review policies and procedures.
  • Advocate for community health navigation needs and supervise, coach, and develop the navigation team.

Quality Assurance Responsibilities:
  • Monitor and evaluate program outcomes and service quality.
  • Maintain relationships and ongoing communication with community partner providers.
  • Conduct internal and external presentations to facilitate strategic partnerships.
  • Develop and revise program policies and procedures for optimal effectiveness.

Administrative Responsibilities:
  • Establish and oversee administrative procedures to meet senior leadership objectives.
  • Work with the Associate Director of Adult Services to develop and monitor program budgets.
  • Prepare and maintain program performance reports for funders and internal reporting.
  • Ensure accurate client and service records in accordance with CASL policies.
  • Participate in team meetings to contribute to program improvements and outreach efforts.

Key Requirements:

To thrive in this role, you should have:

Education/Experience:
  • Bachelor's degree in Social Work, Public Health, or related field from an accredited institution (Master's degree preferred).
  • At least five (5) years of experience in Community Health and/or non-profit organizations, with three (3) years of supervisory and administrative experience.
  • Education, training, and applicable years of experience may be considered in lieu of a degree.

Skills/Knowledge:
  • Proficiency in spoken and written English required; Spanish or Chinese (Cantonese, Mandarin, or Toishanese) a plus.
  • Strong organizational and time management skills, with the ability to prioritize tasks effectively.
  • Excellent analytical and data management skills, coupled with strong decision-making abilities.
  • Proficiency in Microsoft Office (Word, Excel, PowerPoint); Salesforce knowledge is a plus.
  • Exceptional negotiation skills and rapport-building abilities.
  • Proven ability to work with diverse cultural and income groups.

Environment, Scheduling & Physical Demands:
  • Frequent sitting, standing, bending, and walking for extended periods.
  • Ability to travel around the city for in-person meetings and interactions.
  • In-person interactions may be required within healthcare settings.
  • Standard business hours, Monday through Friday, with flexibility for occasional after-hours work.
