HOME CARE and SUPPORT SERVICES REHAB WORKER-Great Job Flexibility!!  Full and Part Time Openings!

 

Home Care and Support Rehabilitation Workers provide services that address their unique needs related to mental health and substance use. They assist individuals who are in need of care outside of inpatient settings, but who may have ongoing needs that, if left unaddressed, could result in hospitalizations during times of crisis, in their own homes or community-based settings. The goal is to strive for independence in activities of daily living while building or bridging interdependence in community social relationships.

This position requires the person to work with both adults and children. Due to the diversity of the population served, the applicant must meet the following requirements:

 

  1. Must be able to pass a Caregiver Background Check 
  2. Willing to complete a Department of Motor Vehicle driving record check AND not have a violation of Reckless Driving and/or Operating Under Influence of Intoxicant or Other Drug in the last 12 months.
  3. Shall be at least 18 years of age, have at least one year of experience as a licensed driver, and hold a current Wisconsin operator's license. [DCF 57.12(2)]
  4. Experience working with people with mental illness is helpful.

JOB PURPOSE

 

The role of a Home Care and Support Services Rehabilitation Worker is to support the well-being of individuals in their own homes or community-based setting. A key component of this job is to assist clients in their social, emotional, and daily life skills development. The goal is to strive for independence in activities of daily living while building or bridging interdependence in community social relationships. Services include crisis intervention, behavior management, and life skills training, whether one-on-one or in a group environment.

 

Rehabilitation Workers often work in teams to include working with a team leader, case managers, and other client selected members. They are respectful to client desires, needs, and preferences. Rehabilitation Workers model skill sets, boundaries, offering of choices, and problem-solving by looking at options. They strive to create a fun and enjoyable atmosphere to create client participation and goal achievement. This person has sensitivity towards cultural, ethnic, and disability issues.

 

DUTIES AND RESPONSIBILITIES

 

  • Help clients create daily routines through use of calendars, tracking tools, schedules, alarms, etc.
  • Teaches and/or provides transportation to mental health appointments.
  • Makes client aware of educational, physical, social, and/or recreational opportunities available
  • Provides verbal prompts when needed for personal hygiene to be acceptable
  • Shows how to keep home safe and sanitary; sets up schedule to address any concerns
  • Provide medication management xervices to include supporting the client in taking his/her medications, increasing the client's understanding and the benefit of medications and awareness of symptoms, and monitoring changes to client's symptoms and side-effects.
  • Monitors clients’ well-being and reports to team members when changes are noted
  • Recognizes & responds to potential crisis and emergency situations
  • Implements care plans and notes results
  • Participates in team meetings, reporting observations, reviewing progress, and makes suggestions
  • Asks for help when unable to manage a situation or general concern
  • Follows agency policy
  • Incorporates new learning into job performance

  • Experience being exposed to people with mental illnesses

 

Knowledge, Skills, and Abilities:

 

  • Ability to provide information to clients in a direct, but respectful manner

  • Ability to break down activities into achievable goals

  • Ability to observe clients’ performance and report clearly to relevant professions

  • Team working skills in order to be able to effectively communicate/interact with other professionals, peers, and community members

  • Ability to research available appropriate client activities in the local area

  • Knows principles of good nutrition and food preparation

  • Time management skills; must be able to be on time to client's homes and/or appointments or meetings. 

  • Self-motivated and able to function without daily supervision

  • Personal resilience to manage own health and well being

  • Rapport building skills of patience, empathy, sense of humor, and a flexible approach

 

 PHYSICAL REQUIREMENTS

 

The work entails being mobile and able to get from one location to another. There is no “hand-on” care, heavy lifting, excessive standing, or bending.  Must have the ability to drive a car.
