JOB SUMMARY

To manage the Permanent Supportive Housing (PSH) program and lead the supportive services team.

ESSENTIAL FUNCTIONS

  1. Facilitates a smooth move-in process for new residents
    1. Receives referrals and conducts interviews for potential residents. Provides choices to applicants by showing available properties. Facilitates lease signing and all related intake paperwork.
    2. Educates residents on housing procedures including paying rent and reporting maintenance problems.
    3. Maintains a waiting list and ensures vacancies are filled within the timeframes required by funders.
  2. Ensures services are delivered by the PSH team with a recovery-oriented, trauma-informed approach
    1. Provides day-to-day supervision of PSH Coordinators to achieve program goals of housing stability, community integration, and increased resources and supports
    2. Provides ongoing training to PSH Coordinators on the principles of Housing First, Recovery Principles, Psychiatric Rehabilitation, Motivational Interviewing, and Trauma-informed Care.
    3. Ensures that development and implementation of Recovery Plans is consistent with organizational values, program goals, and evidence-based practices
    4. Guides staff in providing case management utilizing trauma-informed care, harm reduction, assertive engagement, and a strengths-based approach, in alignment with PSH best practices.
    5. Inspires staff to empower the residents and encourage autonomy and a sense of self-determination.
    6. Oversees coordination of services with internal and external providers
    7. Supports staff in handling difficult or complex cases.
  3. Ensures accreditation and funding source requirements are met
    1. Ensures timely and accurate documentation is completed by the PSH Coordinators (progress notes, recovery plans, DLA-20s, etc.)
    2. Ensures residents are seen at least two times a month in accordance with TDMHSAS requirements.
  4. Administrative Responsibilities:
    1. Coordinates with Property Manager on all property management functions (e.g., leasing, orientation, inspections, maintenance requests, evictions, etc.)
    2. Ensures each resident has an electronic client file containing referral information, up-to-date leases, progress notes, financial verification if required, and other pertinent documentation.
    3. Assists Director in maintaining and reporting monthly housing data and outcomes.
    4. Assists Director in developing and revising policies and procedures.
    5. Complies with TDMHSAS grant requirements for Community Supportive Housing.
  5. Supervisory Responsibilities:
    1. Orients new staff to program mission, goals, policies, and procedures
    2. Oversees job responsibilities of the Lead PSH Coordinator, Peer PSH Coordinator(s) and PSH Coordinator(s) and other personnel related to the PSH program.
    3. Conducts and documents weekly supervision, quarterly work plans, monthly audits on progress notes and service plans and performance reviews of supervised staff.
  6. Public/Staff Relations:
    1. Develops positive working relationships with landlords, neighbors, and community agencies.
    2. Works with MDHA and other agencies, committees and organization as assigned on housing issues.
    3. Utilizes voice and email.
    4. Communicates regularly with other staff to meet needs of members and residents.
    5. Works as a team player with other staff.
  7. Values cultural competence and diversity:
    1. Collaborates with staff, members, and the community to gain the perspectives of others having diverse opinions, abilities, values, beliefs, perspectives, and ethnic or cultural backgrounds.
    2. Takes deliberate steps to increase own cultural competency by attending trainings, events, discussions, workshops, etc.
    3. Is intentional to increase cultural competency within the department.
  8. Other duties as a Member of Park Center Permanent Supportive Housing team:
    1. Leads staff and supervision meetings.
    2. Assists with housing events.
    3. Participates in on-call rotation.
    4. Maintains good attendance and informs supervisor in advance of annual leave or other time off.
    5. Follows all Personnel Policies.
    6. Completes required Relias Learning yearly.
    7. Other duties as assigned.

QUALIFICATIONS

  • Ability to work independently and with a team.
  • Ability to meet deadlines
  • Excellent communication skills.
  • Strong professional and ethical behaviors.
  • Must have Bachelor’s Degree in a human services field.
  • Must have at least 3 years’ experience working in a human services field.
  • At least 1 year of experience in a supervisory role preferred.
  • Must be a CPRP or be willing to be certified
  • Must have a valid Tennessee Driver’s License.

PHYSICAL DEMANDS

While performing the duties of this job, the employee is occasionally required to stand; walk; sit for long periods of time; use hands to fingers, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; talk or hear. The employee must occasionally lift or move up to 10 pounds. Must have visual ability to detect errors.
