Job Description

Job Title: Community Health Engagement Coordinator/Trans Mobilization FLSA Status: Non - Exempt

Department: Prevention Services Location: 1348 Bainbridge Street

Job Summary: The Community Health Engagement Coordinator is a full time position responsible for engagement, outreach, service support, and referrals for Trans Women of color in Philadelphia. The main goal of this position is to educate, refer and support Trans Women of color in the process of linkage to a variety of supportive services but most specifically ongoing health and wellness activities, HIV testing, medical care and Pre-Exposure Prophylaxis (PrEP).

Communication: Good oral and written communication skills are required for this position. The person in this position must be able to communicate effectively with the clients, staff, and others; and contribute to a positive environment.

Key Responsibilities:

● Works with the Prevention Program Managers and other entities to develop and disseminate, and complete initial and ongoing community needs assessments, focus groups and community member interviews to evaluate the needs of the primary population.

● Facilitates weekly Support Groups for Trans Women of Color, providing health education, resource sharing, in a safe place that is free of judgement that encourages empowerment and growth.

● Uses information gathered from the Community Advisory Board, community assessments, focus groups, surveys, and community member interviews to structure the program

● Works with Mazzoni Center staff and other entities as needed to create promotional materials and identify promotion opportunities

● Spearheads yet works closely with the Prevention Program Managers to provide outreach orientation and training outreach staff and volunteers

● Identifies innovative and effective means to engage and recruit individuals of the primary population for Mazzoni Center programs

● Collaborate with internal and external entities to increase the capacity and the success of outreach to the primary population

● Spearheads and works closely with the Community Engagement Manager in the identification, evaluation and scheduling of outreach venues/locations that will increase engagement in the primary populations

● Perform street, club, community, smartphone application and internet outreach to disseminate information and engage in conversations with the primary population

● Promotes, makes referrals to and supports in the linkage of a variety of services and opportunities to the primary population, most specifically health and wellness activities, STI testing, condom availability, condom usage, PrEP, PEP, syringe exchange programs, medical care, and other services/programs

● Works with a variety of internal and/or external entities to create a safe, gender affirming space where the primary population may receive information and prevention services

● Works to provide or find referrals to activities that reduce HIV stigma for the primary population

● Works to provide or find referrals to health and wellness activities, health and wellness groups, and/or health and wellness events for the primary population

● Works to provide or find referrals to activities that will address social determinants of health to address barriers to sexual health care for the primary population

● Collect data at testing sites and produce quantitative and qualitative reports as required by the Prevention Program Manager and the Prevention Services Director

● Collect data from online/smartphone application outreach and produce quantitative and qualitative reports as required by the Prevention Program Manager and the Prevention Services Director

● Performs data entry and reporting

● Meets program goals

● Supports other team members when needed

● Perform all other duties as assigned

Other

● Protect client information and records at all times in compliance with agency Confidentiality Policy and Pennsylvania Act 148

● Participate in weekly supervision with Community Engagement Manager

● Participate in agency meetings, staff in-services, and external trainings as required

● Maintains excellent communication with Community Engagement Manager, and the Community Health Engagement Team.

● Perform other duties as assigned by Supervisor

● Work offsite, outside of center city Philadelphia during late and/or weekend hours with minimum supervision

Education, Training, License and Certification, Skills and Experience:

● Commitment to the mission of Mazzoni Center required

● High School Diploma or GED required

● Experience working with Trans Communities of Color the MSM of color communities required

● Strong understanding of the Trans Women of color communities of Philadelphia required

● Flexible schedule to work daytime, afternoon, evening, late night, and weekend hours required

● Strong understanding of LGBTQ+ cultural competency, specifically trans cultural competency required

● Strong outreach skills, customer service skills and public relations skills required

● Ability to establish and maintain community partnerships required

● Ability to coordinate outreach workers/volunteers and oversee a schedule

● Ability to provide training to outreach workers, volunteers and other entities

● Comfortable working with an extremely wide range of diverse individuals

● Able to work well within a team environment with many different public health professionals and direct service staff in a variety of disciplines

● Good organizational skills and good follow through

● Good written and oral communication skills

● Working knowledge of HIV/AIDS prevention and community resources preferred

● Experience in facilitating meetings and working with community organizations preferred

● Attention to detail, excellent communications and organizational skills, and the ability to organize effectively preferred

● Computer literate with knowledge of Windows and Microsoft Office preferred

Clearances:

1. Background checks are required, including Criminal Background Clearance, Child Abuse Clearance, and FBI Clearance.

Physical Requirements:

1. The ability to stand, sit and walk for extended periods of time

2. The ability to lift objects greater than 35 pounds

3. The ability to communicate using written and spoken words

4. Must demonstrate visual and auditory acuity

5. Must have all necessary vaccinations

6. Annual PPD
