Salary: $21.94 per hour

Community Youth Services’ Foster Care Program is looking for compassionate and dedicated individuals to join our team as Clinical Case Manager. The Clinical Case Manager is a pivotal figure in the Foster Care Program, providing comprehensive case management for youth in foster care and their families. This role is integral to the initial assessment and placement of youth, ensuring that each youth is matched with a nurturing and supportive environment. Through collaboration with a multidisciplinary team, the Clinical Case Manager develops and oversees personalized treatment plans, advocating for the necessary support and services to maintain stability in the home. The impact of this position is profound, as it extends beyond immediate care to include crisis intervention, therapeutic services, and advocacy in court settings. By coordinating with various stakeholders, the Clinical Case Manager ensures that each youth’s journey towards permanency is carefully guided and supported. The commitment to fostering positive outcomes through structured reviews and goal-oriented plans makes this role essential in shaping the lives of young individuals and their families, ultimately contributing to a healthier, more resilient community.

Qualifications:

  • Bachelor’s degree in Social Work or closely related field and 2 years’ experience in foster care, family systems, residential treatment and/or with children and families who have been traumatized by abuse, neglect and/or domestic violence.
  • Valid TB test results within the previous 12 months prior to hire or the ability to obtain a negative TB test within 30 days of employment.
  • The ability to handle confidential materials in a professional manner.

Direct Services:

  • Collaborate with child welfare director and caseworkers on referrals.
  • Collect data for informed placement decisions.
  • Screen foster families with program staff.
  • Offer crisis intervention for family stability.
  • Arrange necessary transportation for participants.
  • Lead a treatment team for youth and family support.
  • Deliver therapeutic services to individuals, groups and families

Case Management:

  • Coordinate comprehensive services with multiple stakeholders.
  • Support families to ensure youth stability.
  • Advocate for sufficient support for youth and families.
  • Share on-call duties for crisis response.
  • Conduct intake assessments for placement suitability.
  • Lead Child and Family Team meetings for treatment planning.
  • Represent youth in court as required.
  • Conduct monthly case reviews to evaluate and update plans.
  • Support permanency planning for youth with DCYF.
  • Organize prosocial activities and case management services.
  • Ensure achievement of performance outcomes.

Agency Qualifications:

  1. Successfully clear a thorough background check with no history of causing harm, particularly to children or vulnerable adults.
  2. Maintain a clean criminal record, demonstrating professional and personal integrity.
  3. Hold a valid Washington State Driver's License with at least five years of driving experience and a good driving record for the past three years.
  4. Provide proof of auto insurance with coverage of $100,000 or more.
  5. Show cultural sensitivity and the ability to respond to the diverse needs of the organization’s community.
  6. Acquire CPR/1st Aid/Bloodborne Pathogen certification within 30 days of employment (required before starting some positions).
  7. Possess strong interpersonal skills and the ability to collaborate effectively within a team.
